How does GLP-1 work for weight loss?
GLP-1 receptor agonists are a breakthrough class of medications originally designed to manage type 2 diabetes. Today, they’re also widely used to support safe and effective weight loss. These medications work by mimicking a hormone in your body called glucagon-like peptide-1 (GLP-1), which is naturally released after eating to help regulate blood sugar and appetite. Popular GLP-1 medications include Ozempic, Wegovy, Mounjaro, Saxenda, Trulicity, and Rybelsus, each offering unique benefits depending on your individual goals and medical history.
First approved in 2005, the earliest GLP-1 drug—exenatide—was derived from a peptide found in the saliva of the Gila monster. Since then, the class has evolved to include longer-acting, more convenient formulations, including once-weekly injections and even an oral option. These medications support healthy metabolic function by stimulating insulin release when blood sugar levels rise, reducing excess glucagon (a hormone that raises blood sugar), slowing gastric emptying to prolong satiety, and acting on brain centers to reduce appetite. Some GLP-1 agents, like semaglutide and liraglutide, have also shown cardiovascular benefits in people with existing heart disease.
GLP-1 medications are an excellent choice for individuals with type 2 diabetes who are not achieving their goals on oral medications alone, as well as for adults with obesity or overweight who are struggling to lose weight through diet and exercise. They’re also ideal for individuals with prediabetes or metabolic syndrome who are looking for medically supervised support in reducing their long-term health risks. Patients with a history of cardiovascular disease may benefit from specific GLP-1s that have been shown to lower the risk of heart attack and stroke.
Like any medication, GLP-1s are not suitable for everyone. These medications should be avoided in patients with a personal or family history of medullary thyroid cancer, Multiple Endocrine Neoplasia syndrome type 2 (MEN2), or those with significant gastrointestinal conditions such as gastroparesis. Caution is advised in people with a history of pancreatitis, gallbladder disease, or renal impairment. They are also not recommended during pregnancy or while breastfeeding. When starting GLP-1 therapy, it’s important to be aware of potential side effects, which are typically mild and include nausea, constipation, diarrhea, or decreased appetite. These symptoms often improve within the first few weeks of use. Rare but more serious risks include pancreatitis, gallbladder issues, and thyroid tumors (observed in animal studies).
Administration of GLP-1 medications is typically through a once-weekly injection, though semaglutide is also available as an oral tablet. Most patients notice appetite suppression within one to two weeks of starting treatment, and weight loss of 10–20% is achievable over several months when paired with a healthy lifestyle.
